Maximum strength Desitin works like a charm -- that 40% zinc oxide is the key -- but it also contains cod liver oil, which I think is responsible for much of the nasty fishy odor that seems worse in some batches than others. Note that it also contains talc, which is primarily a concern in loose powders, but can be a concern for some in any form as there are some studies linking it to cancer -- you'll need to read those with a critical eye for yourself, and judge if they apply to diaper cream.
I thought I was stuck with Desitin if I wanted 40% zinc oxide, but I recently found Canus Li'l Goat's Milk 40% Zinc Oxide Ointment 10 oz (248 g) -- no stink, and it works at least as well as Desitin for us!

This is way cheaper than buying it in tubes[...]. Stuff works wonders too. Rashes start to dissapper within hours. The only thing i dont like is the lack of smell. I made the mistake of buying the original , instead of the scented one. This one smells mediciny. But its ok. Stuff works great and these 2 containers will last me about 1 year.Customer Review: Not as thick

Desitin worked great on my daughter's diaper rash. However, the jars I bought on Amazon was not as thick as the jar from Target. There was also some oil separation. Don't know if this affects the efficacy.Customer Review: May work on Rash, but STINKS!

Goodness gracious but this stuff stinks to HIGH heaven! I use it as a preventive measurement since my son rarely suffers from diaper rash, but good googly moogly, this stuff smells worse than poop. I mean, you'd think if Johnson & Johnson were going to add a fragrance they might have thought to add one that didn't smell so rancid.
Other than that, I do like the nice thick consistency, but with one drawback: it is so thick that it really adheres to my baby's bottom when I am trying to clean him off and leaves an opaque film behind. In some cases it helps the poop adhere to my son's butt, too!
You get a lot of product with these two jars, but I honestly don't think my nose can tolerate the smell for much longer. If it were easier to clean off, I might reconsider....NAH! No more Desitin for this household.
****I am updating my review because, though stinky, nothing else keeps my six month old son's butt rash-free better than Desitin (including the recommended Butt Paste). The smell is a small price to pay for my son's comfort.
