I am a cardiologist and have acquired a good number of stethoscopes, including electronic models, over the years. I recently purchased a Thinklabs ds32a and found it to be the best one I've used yet. My only regret is that I didn't see it at the ACC meeting before i bought a Littmann 3100 (which was a disappointment when I used it on real patients).
Compared to the Littmann, the overall sounds is much cleaner and clearer. The higher frequencies are not rolled off and I can hear grade 1 systolic murmurs at the "acoustic" setting (non amplified level) that are barely audible at high amplification on the Littmann. Mechanical artificial valve closing is heard as a true click rather than a clunk. High pitched bruits and aortic regurgitation murmurs are much clearer.
The low frequencies are also clean, detailed and extended - unlike the resonant, smeared, "boom box" sound of the Littmann. S3 and S4 are easy to distinguish from split S2 and S1 by their natural pitch.
Another advantage is its adjustability to get optimal performance. Ambient noise reduction can be turned off when in a quiet environment, improving the sound. The Littmanns (and others) don't have this option and noise reduction will always degrade the primary sound to some degree. Loudness can be quickly changed on the fly simply by changing the manual pressure on the scope head, bringing out subtle sounds.
The only minor downside is that there is a small learning curve for the more complex control of the sound and the need to use 2 hands. However, after a couple of days it became completely natural. I recommend this stethoscope most highly.
